This combination therapy consists of itacitinib, an oral small-molecule Janus kinase 1 (JAK1) inhibitor, and axicabtagene ciloleucel (axi-cel), an autologous anti-CD19 chimeric antigen receptor (CAR) T-cell therapy. The regimen is being investigated by the H. Lee Moffitt Cancer Center and Research Institute for the treatment of diffuse large B-cell lymphoma (DLBCL), specifically in patients with high systemic inflammation. Itacitinib is used as a pre-modulation therapy, administered starting after apheresis and continuing through the CAR-T infusion period, to potentially reduce inflammatory markers (such as ferritin and CRP) and improve the safety and efficacy of the CAR-T therapy by mitigating cytokine release syndrome (CRS) or other inflammation-driven toxicities.
