Itacitinib + corticosteroids is a combination therapy evaluated primarily for the treatment of acute graft-versus-host disease (aGVHD) following allogeneic hematopoietic stem cell transplantation. Itacitinib is an oral, selective Janus kinase 1 (JAK1) inhibitor that modulates inflammatory cytokine signaling pathways implicated in the pathogenesis of aGVHD. Corticosteroids are broad-spectrum immunosuppressive agents commonly used as first-line therapy for aGVHD. The combination aims to enhance clinical response while permitting earlier corticosteroid tapering. This regimen has been tested in phase 1 and phase 3 trials as first-line or subsequent treatment in steroid-naive or steroid-refractory aGVHD. Itacitinib is being developed by Incyte.
