ITM-74 is a diagnostic radiopharmaceutical candidate developed by Isotope Technologies Munich (ITM). It consists of a targeting ligand specific for Fibroblast Activation Protein (FAP) labeled with the positron-emitting radioisotope Gallium-68 (Ga-68). FAP is a cell-surface serine protease highly overexpressed on cancer-associated fibroblasts (CAFs) in the stroma of most epithelial tumors, as well as in non-malignant conditions involving active tissue remodeling, such as pulmonary fibrosis, rheumatoid arthritis, and cardiovascular inflammation. ITM-74 is designed for use with Positron Emission Tomography (PET) imaging to visualize FAP expression, serving as a diagnostic tool for patient stratification in theranostic applications (often paired with the therapeutic counterpart ITM-71) and for the assessment of disease activity in oncology, fibrosis, and inflammation.
