iTOL-100 is a synthetic immunomodulatory platform developed by iTolerance. It consists of a synthetic form of the naturally occurring protein Fas ligand (FasL) combined with a synthetic biotin polyethylene glycol (PEG) microgel, also referred to as SA-FasL microgel. The technology is designed to induce durable and localized immune tolerance by binding FasL to the Fas receptor on activated T-cells, triggering apoptosis and expanding regulatory T-cells at the site of implantation. This mechanism prevents immune rejection of transplanted cells or tissues without requiring chronic systemic immunosuppression. The primary indication under development is enabling allogeneic cell therapy for Type 1 Diabetes by allowing insulin-producing stem cell-derived pancreatic islets or organoids to be implanted and function long-term in patients. Preclinical studies have demonstrated its ability to establish localized immune privilege for implanted tissues, with potential applications in other areas such as liver failure and large organ transplantation[1][3][5][6][7].
