This combination therapy, investigated by Ain Shams University, involves the addition of the oral antifungal agent itraconazole to standard platinum-based chemotherapy (typically cisplatin or carboplatin plus gemcitabine) for the treatment of advanced non-small cell lung cancer (NSCLC). Itraconazole is repurposed here for its potent anti-angiogenic properties, which include the inhibition of vascular endothelial growth factor (VEGF) and fibroblast growth factor (FGF) signaling, as well as the Hedgehog and AKT/mTOR pathways. By targeting multiple angiogenic and survival pathways, itraconazole aims to enhance the efficacy of cytotoxic chemotherapy. Clinical results from a Phase 2 trial (NCT03664115) demonstrated improved objective response rates and progression-free survival in metastatic NSCLC patients compared to chemotherapy alone.
