This combination therapy consists of the systemic antifungal agent itraconazole and the oral retinoid isotretinoin. Itraconazole is a triazole antifungal that inhibits the fungal cytochrome P450-dependent enzyme lanosterol 14-alpha-demethylase, thereby blocking the synthesis of ergosterol, an essential component of fungal cell membranes. Isotretinoin is a vitamin A derivative (retinoid) that modifies keratinization and reduces sebum production, potentially altering the skin microenvironment to prevent the persistence or recurrence of fungal elements. The combination is being investigated by Multan Medical And Dental College as a treatment for resistant dermatophytosis caused by Trichophyton species, particularly in cases where standard antifungal monotherapy has failed to achieve complete clearance or prevent relapse.
