Itraconazole + Ofloxacin + Ornidazole + Clobetasol Propionate (brand name: OLOX-MX) is an approved topical combination cream developed and manufactured by Thrift Pharmaceuticals. This broad-spectrum dermatological formulation is designed to treat mixed skin infections by addressing fungal, aerobic bacterial, anaerobic bacterial, and inflammatory components simultaneously. Itraconazole, a triazole antifungal, inhibits fungal cytochrome P450-dependent lanosterol 14-alpha-demethylase, thereby impairing ergosterol synthesis and compromising fungal cell membrane integrity. Ofloxacin, a fluoroquinolone antibiotic, inhibits bacterial DNA gyrase and topoisomerase IV, preventing DNA replication and transcription. Ornidazole, a nitroimidazole antibiotic, undergoes reductive activation in anaerobic microorganisms to produce reactive intermediates that disrupt microbial DNA. Clobetasol propionate, a potent topical corticosteroid, acts as a glucocorticoid receptor agonist to suppress the synthesis of inflammatory mediators. The cream is indicated for conditions such as athlete's foot, ringworm, jock itch, and seborrheic dermatitis where concurrent infection and inflammation are present.
