Overview

ITRI-E-212 is a novel, highly selective small molecule inhibitor of Rho-associated protein kinase 2 (ROCK2), developed as a topical ophthalmic agent for lowering intraocular pressure (IOP) in glaucoma and ocular hypertension. It is based on an amino-isoquinoline scaffold, selected for high inhibitory activity toward ROCK2 and high specificity within AGC kinases, with favorable solubility and drug-loading characteristics for eye drop formulations. In animal models, topical administration significantly enhances fluid outflow through the trabecular meshwork, reduces IOP by approximately 25–29%, and demonstrates a favorable pharmacokinetic profile with sustained ROCK2 inhibition and minimal, transient conjunctival hyperemia. Preclinical studies indicate potential neuroprotective effects, including suppression of oxidative stress, prevention of blood-retinal barrier disruption, and mitigation of neuroinflammation, with increased retinal ganglion cell survival and reduction of apoptosis. The primary indication is glaucoma, though other optic neuropathies may also be considered[1][2][3][5].
