Overview

This is a combination regimen consisting of four agents: - **Ivonescimab (AK112/SMT112/Idafang/evoximab):** A first-in-class bispecific monoclonal antibody that targets both programmed death-1 (PD-1) and vascular endothelial growth factor A (VEGF-A). It blocks PD-1-mediated immunosuppression and inhibits VEGF-driven angiogenesis in the tumor microenvironment. Developed to enhance anti-tumor immune responses and inhibit tumor blood vessel formation[1][2][5]. - **Cadonilimab:** A global first-in-class bispecific antibody targeting PD-1 and cytotoxic T lymphocyte-associated antigen 4 (CTLA-4). It is designed to improve immunotherapy efficacy by dual checkpoint inhibition with reduced toxicity due to its unique tetravalent symmetric structure. Cadonilimab has been approved for recurrent/metastatic cervical cancer and gastric/gastroesophageal junction adenocarcinoma in China[6]. - **Nab-paclitaxel:** An albumin-bound formulation of paclitaxel used as a chemotherapeutic agent. It disrupts microtubule function during cell division. - **Gemcitabine:** A nucleoside analog used as chemotherapy; it inhibits DNA synthesis leading to apoptosis in rapidly dividing cells. This combination is being investigated primarily for advanced solid tumors including non-small cell lung cancer (NSCLC), leveraging synergistic effects between immune checkpoint blockade and cytotoxic chemotherapy[8].
