This combination therapy consists of ivonescimab (AK112), a first-in-class humanized tetravalent bispecific antibody, administered in conjunction with the chemotherapy agents carboplatin and albumin-bound paclitaxel (nab-paclitaxel). Ivonescimab simultaneously targets and inhibits programmed cell death protein 1 (PD-1) and vascular endothelial growth factor A (VEGF-A). By blocking the PD-1/PD-L1 pathway, it restores anti-tumor T-cell activity, while its inhibition of VEGF-A suppresses tumor angiogenesis and promotes vascular normalization. The addition of carboplatin, a DNA-alkylating agent, and nab-paclitaxel, a microtubule stabilizer, provides direct cytotoxic effects. This specific regimen is being evaluated as a neoadjuvant treatment for patients with early or locally advanced triple-negative breast cancer (TNBC) to improve pathological complete response (pCR) rates and long-term survival outcomes.
