Overview

Ivonescimab (AK112, SMT112) is a first-in-class, humanized tetravalent bispecific monoclonal antibody that targets both programmed death 1 (PD-1) and vascular endothelial growth factor A (VEGF-A). It is designed to simultaneously block PD-1-mediated immunosuppression and inhibit VEGF-driven angiogenesis in the tumor microenvironment. This dual mechanism aims to enhance anti-tumor immune responses while suppressing tumor blood vessel formation. Ivonescimab has been approved in China for use in combination with chemotherapy for EGFR-mutated locally advanced or metastatic non-squamous non-small cell lung cancer (NSCLC) after tyrosine kinase inhibitor therapy[1][2][6]. Nab-paclitaxel is an albumin-bound formulation of paclitaxel, a microtubule inhibitor used as a chemotherapeutic agent. Gemcitabine is a nucleoside analog that inhibits DNA synthesis and is widely used as chemotherapy for various solid tumors. The combination of these three agents leverages complementary mechanisms—immune checkpoint inhibition, antiangiogenesis, and cytotoxicity—to treat advanced solid tumors.
