Overview

Ivonescimab + short-course radiotherapy is a combination regimen being investigated for the neoadjuvant treatment of locally advanced rectal cancer (LARC). Ivonescimab (AK112) is a first-in-class humanized tetravalent bispecific antibody that simultaneously targets programmed cell death protein 1 (PD-1) and vascular endothelial growth factor A (VEGF-A). This dual blockade is designed to enhance anti-tumor immune responses while inhibiting angiogenesis and normalizing tumor vasculature. In this regimen, neoadjuvant short-course radiotherapy (SCRT) is administered sequentially followed by ivonescimab, often in combination with CapeOX chemotherapy (oxaliplatin and capecitabine). The goal of this approach is to leverage the immunogenic cell death induced by radiotherapy to prime the immune system, potentially increasing the efficacy of the bispecific checkpoint inhibitor and improving pathological complete response rates.
