IW-6118 is an orally active small molecule inhibitor of fatty acid amide hydrolase (FAAH), an enzyme that degrades endocannabinoid neurotransmitters such as anandamide. By inhibiting FAAH, IW-6118 increases levels of these neurotransmitters, which can influence pain, inflammation, sleep, and eating behaviors. It was developed by Ironwood Pharmaceuticals primarily for analgesic and anti-inflammatory indications. The drug has been investigated in clinical trials for dental pain (postoperative pain after third molar extraction) but showed limited efficacy with acute dosing. Despite some clinical development including a Phase 2 trial completed in dental pain, the development status is currently pending or discontinued for inflammation and pain indications. FAAH inhibitors like IW-6118 are considered to have potential benefits without the adverse effects typical of direct cannabinoid receptor agonists.[1][5][6][7][8]
