Overview

IZ10023 is an oral small molecule pharmacokinetic (PK) enhancer being developed by Izumi Biosciences for the treatment of Amyotrophic Lateral Sclerosis (ALS). It functions as a dual inhibitor of two drug efflux pumps, P-glycoprotein (P-gp) and Breast Cancer Resistance Protein (BCRP), which are over-expressed in the diseased spinal cords and blood-brain barrier of ALS patients. These pumps actively remove riluzole—the standard-of-care treatment—from the central nervous system (CNS), thereby limiting its therapeutic efficacy. By blocking these efflux mechanisms, IZ10023 is designed to maintain effective concentrations of riluzole in the brain and spinal cord. The development of IZ10023 has been supported by the U.S. Department of Defense (DoD) Congressionally Directed Medical Research Programs (CDMRP) and is currently in the preclinical/IND-enabling stage.
