Izalontamab + AP or TP is a combination therapy regimen being developed by Sichuan Baili Pharmaceutical Co., Ltd. for the treatment of non-small cell lung cancer (NSCLC). The regimen consists of izalontamab (SI-B001), a first-in-class bispecific antibody targeting the epidermal growth factor receptor (EGFR) and human epidermal growth factor receptor 3 (HER3), administered in conjunction with standard-of-care chemotherapy. The chemotherapy component consists of either the AP regimen (pemetrexed combined with a platinum agent such as cisplatin or carboplatin) or the TP regimen (paclitaxel combined with a platinum agent). Izalontamab is designed to simultaneously inhibit ligand-induced EGFR homodimerization and EGFR/HER3 heterodimerization, thereby blocking downstream oncogenic signaling pathways. The chemotherapy agents provide complementary cytotoxic activity through DNA cross-linking, inhibition of thymidylate synthase, or microtubule stabilization. This combination is primarily being evaluated in Phase II clinical trials for patients with EGFR and ALK wild-type recurrent or metastatic NSCLC who have progressed on prior anti-PD-1/PD-L1 therapies.
