J-Up is a dietary supplement marketed for sexual enhancement and the treatment of erectile dysfunction. Although promoted as a natural herbal product, laboratory analysis by regulatory agencies such as the FDA has revealed that J-Up contains undeclared sildenafil. Sildenafil is a potent phosphodiesterase type 5 (PDE5) inhibitor that works by increasing blood flow to the penis through the relaxation of smooth muscle. The presence of this undeclared pharmaceutical ingredient makes J-Up an unapproved drug, posing serious safety risks, especially for patients using organic nitrates (such as nitroglycerin), as the interaction can cause life-threatening hypotension. The product was the subject of a public notification by the FDA in 2014 warning consumers against its use.
