The JANUS Tacrolimus-eluting Carbostent is a polymer-free drug-eluting stent (DES) developed by Sorin Group (CID) for the treatment of coronary artery disease. It features a stainless steel platform coated with a thromboresistant pyrolytic carbon film (Carbofilm) and incorporates deep reservoirs on its abluminal surface. These reservoirs are loaded with tacrolimus, a small molecule immunosuppressant that acts as an antiproliferative agent to inhibit smooth muscle cell proliferation and prevent restenosis. The design allows for targeted drug delivery directly to the vessel wall without the need for a polymer carrier, potentially reducing long-term inflammatory responses associated with polymer-based stents. The device has been evaluated in clinical trials such as JUPITER I and JUPITER II.
