JCARH125 + anakinra is an investigational combination therapy being evaluated for the treatment of relapsed or refractory multiple myeloma. JCARH125 (oricabtagene autoleucel) is an autologous chimeric antigen receptor (CAR) T-cell therapy targeting B-cell maturation antigen (BCMA), which is highly expressed on malignant plasma cells. The therapy utilizes a lentiviral vector to transduce T-cells with a construct containing a fully human scFv, a 4-1BB costimulatory domain, and a CD3ζ activation domain. Anakinra is a recombinant interleukin-1 (IL-1) receptor antagonist. In this combination, anakinra is administered prophylactically to mitigate cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS), common and potentially severe toxicities associated with CAR T-cell activation. This approach is being studied in the Phase 1/2 EVOLVE trial (NCT03430011) to improve the safety and tolerability of BCMA-targeted CAR T-cell therapy.
