Jiawei Sini Tang (加味四逆汤) is a traditional Chinese medicine (TCM) herbal formula. It is an augmented or modified version of the classic "Sini Tang" (Four Reversal Decoction), designed to treat conditions associated with Yang deficiency and internal cold, particularly in cases where additional symptoms or patterns are present that require modification of the base formula. The original Sini Tang consists of processed Aconite root (Fu Zi), dried ginger (Gan Jiang), and licorice root (Zhi Gan Cao). Jiawei ("augmented") versions add other herbs tailored to specific clinical presentations. Mechanistically, Jiawei Sini Tang is used in TCM to warm the interior, rescue devastated Yang, dispel cold, and support heart and kidney Yang. It may be prescribed for chronic heart failure with heart-kidney Yang deficiency syndrome[2], as well as other disorders characterized by cold extremities, fatigue, abdominal pain/diarrhea due to internal cold or collapse of Yang Qi. The precise mechanism from a biomedical perspective remains unclear; its effects are attributed in TCM theory to modulation of Qi and blood circulation through warming actions on the body’s core systems.
