Jin Bu Huan is a traditional Chinese herbal product, most commonly sold as Jin Bu Huan Anodyne Tablets, used primarily as a **sedative** and **analgesic**. Its principal active ingredient, **levo-tetrahydropalmatine (L-THP)**, is a plant alkaloid with potent neuroactive properties, producing analgesia, sedation, antispasmodic, and hypnotic effects. The actual plant source is often **Stephania** species (due to L-THP content), although the packaging often misidentifies the source as **Polygala** species. Mechanistically, L-THP is known to exhibit **dopamine receptor antagonism** with no appreciable affinity for opiate receptors and is resistant to naloxone reversal. Jin Bu Huan has been associated with severe toxicity including **central nervous system depression, respiratory depression, life-threatening bradycardia** in children after acute ingestion, and **hepatotoxicity (acute hepatitis)** after chronic adult use. It is not approved by any major regulatory agency as a pharmaceutical, is often labeled as a dietary supplement, and has been implicated in several poisoning cases in the United States.[1][2][3][4][5]
