JNJ-26366821 is a synthetic, PEGylated peptide that acts as a thrombopoietin (TPO) mimetic. It was developed by Johnson & Johnson (Janssen) for the treatment of thrombocytopenia and chemotherapy-induced anemia. The drug binds to and activates the c-MPL (TPO receptor), stimulating megakaryopoiesis and increasing platelet production without sequence homology to endogenous TPO[1][6][8]. Preclinical studies have shown efficacy in promoting hematopoietic recovery after radiation injury, enhancing liver regeneration after acetaminophen-induced injury, and stimulating platelet production in both healthy individuals and those with myeloid malignancies such as myelodysplastic syndrome (MDS) or acute myeloid leukemia (AML), without promoting malignant cell proliferation[1][3][5][6]. The drug has been evaluated in phase 2 clinical trials for thrombocytopenia and anemia, with additional preclinical research in MDS/AML[2][4][7].
