AB-178 (also known as JNJ-63709178) is a humanized bispecific IgG4 antibody developed by Janssen Research & Development for the treatment of acute myeloid leukemia (AML). It is designed to redirect T cells to eliminate leukemic cells by simultaneously binding to the CD123 antigen (the alpha chain of the interleukin-3 receptor), which is overexpressed on AML blasts and leukemic stem cells, and the CD3 epsilon chain on T cells. This dual binding triggers T-cell activation and the subsequent release of cytotoxic proteins to kill the targeted cancer cells. The molecule utilizes a full-length IgG4 scaffold, which is intended to provide a longer serum half-life compared to smaller bispecific formats. Although it showed potent activity in preclinical models and entered Phase 1 clinical trials, the development of JNJ-63709178 was ultimately terminated by the sponsor.
