JNJ-6420 is a first-in-class anti-hK2 antibody-based targeted radiotherapy developed for the treatment of metastatic castration-resistant prostate cancer (mCRPC). It consists of a monoclonal antibody that specifically targets human kallikrein 2 (hK2), a protein encoded by the KLK2 gene and expressed on the surface of prostate cancer cells with limited expression elsewhere. The antibody is conjugated to actinium-225, a high-energy, short-range alpha-particle emitter. This design enables selective delivery of potent alpha radiation directly to prostate cancer cells, aiming to maximize tumor cell kill while minimizing off-target effects. Early clinical studies have shown promising efficacy with deep and durable responses in heavily pretreated mCRPC patients; however, significant safety concerns have been noted, including thrombocytopenia, interstitial lung disease (ILD), and treatment-related deaths at higher cumulative doses[1][3][5][6].
