JNJ-64400141 is an antibody-drug conjugate (ADC) developed by Janssen that targets ectonucleotide pyrophosphatase/phosphodiesterase family member 3 (ENPP3), also known as CD203c. ENPP3 is a cell-surface glycoprotein that is highly and specifically expressed in certain malignancies, particularly clear cell renal cell carcinoma (ccRCC), while showing minimal expression in normal tissues. The ADC consists of a human IgG1 monoclonal antibody directed against ENPP3, linked to a potent cytotoxic microtubule-disrupting payload. Upon binding to the extracellular domain of ENPP3 on the tumor cell surface, the ADC is internalized via endocytosis and transported to lysosomes, where the cytotoxic agent is released. The payload then binds to tubulin, inhibiting microtubule polymerization and causing cell cycle arrest in the G2/M phase, ultimately leading to apoptosis. JNJ-64400141 has been evaluated in Phase 1 clinical trials for the treatment of patients with advanced or metastatic solid tumors, with a primary focus on ENPP3-positive renal cell carcinoma.
