JNJ-77730614 is an orally bioavailable small molecule antagonist of the P2X7 receptor, currently in Phase 2 clinical development by Janssen Research & Development for the treatment of Major Depressive Disorder (MDD). The P2X7 receptor is an ATP-gated cation channel primarily expressed on immune cells, including microglia in the central nervous system. Its activation by extracellular ATP, often released during cellular stress or injury, triggers the NLRP3 inflammasome pathway, leading to the maturation and release of pro-inflammatory cytokines such as interleukin-1 beta (IL-1β) and interleukin-18 (IL-18). By inhibiting P2X7, JNJ-77730614 aims to attenuate neuroinflammation, which is increasingly recognized as a key driver in the pathophysiology of depression, particularly in patients who exhibit systemic inflammatory markers and poor response to traditional monoaminergic antidepressants.
