JNJ-86974680 is an investigational small molecule drug that acts as a selective antagonist of the adenosine A2A receptor (A2aR; ADORA2A). It is being developed as an immune checkpoint inhibitor with potential immunomodulating and antineoplastic activities. By blocking the A2a receptor on immune cells, JNJ-86974680 inhibits adenosine-mediated immunosuppression in the tumor microenvironment. This mechanism may restore or enhance T-cell activity against tumors and improve responses to immunotherapy in cancers such as advanced non-small cell lung cancer (NSCLC), including cases resistant to anti-PD1 therapy. The drug is currently being evaluated in phase 1 clinical trials both as monotherapy and in combination with cetrelimab (an anti-PD1 antibody) and radiotherapy[2][3][4][6][9].
