Overview

**Josamycin** is a macrolide antibiotic produced by *Streptomyces narbonensis* (var. *josamyceticus*) and used for the treatment of various susceptible bacterial infections, including respiratory tract infections, skin infections, otorhinological (ear/nose/throat) infections, urinary tract infections, biliary tract infection, breast infection, dental and oral soft tissue infections, genital infection, and some sexually transmitted diseases[1][4][6][8]. It acts by binding reversibly to the 50S subunit of the bacterial ribosome and inhibiting protein biosynthesis through blockage of transpeptidation/translocation reactions. This results in inhibition of cell growth; its action is mainly bacteriostatic but can be bactericidal at high concentrations[1][5]. It has a broad spectrum of activity against many Gram-positive bacteria as well as some Gram-negative bacteria and atypical pathogens such as mycoplasmas, spirochetes, chlamydias and rickettsias[5][6]. Unlike erythromycin it does not induce macrolide resistance in staphylococci and appears to have fewer gastrointestinal side effects[7].
