JTS-653 is a potent and selective small molecule antagonist of the transient receptor potential vanilloid 1 (TRPV1) channel, currently under development by Japan Tobacco Inc. for the treatment of neuropathic pain. TRPV1 is a non-selective cation channel primarily expressed in peripheral sensory neurons, where it plays a critical role in the detection of noxious heat and the mediation of inflammatory and neuropathic pain. By inhibiting TRPV1, JTS-653 aims to reduce the hypersensitivity and chronic pain associated with conditions such as post-herpetic neuralgia. Clinical development is currently focused on oral administration in patients with post-herpetic neuralgia who have experienced pain for at least three months following the onset of herpes zoster.
