Juan Bi Pill + methotrexate is a combination therapy consisting of Yi Shen Juan Bi Pill (YSJB), a traditional Chinese herbal compound, and methotrexate (MTX), a conventional disease-modifying antirheumatic drug. This combination has been studied primarily for the treatment of rheumatoid arthritis (RA). Methotrexate acts as an anchor drug in RA management by inhibiting osteoclastogenesis and inflammation. Yi Shen Juan Bi Pill is composed of 20 medicinal herbs and has been used extensively in China to treat RA, particularly in patients with kidney deficiency patterns according to traditional Chinese medicine. Mechanistically, YSJB modulates immune responses by regulating T cell phenotypes—upregulating regulatory T cells (Tregs) while downregulating Th1 and Th17 cells—and influences cytokine production such as IL-6, IL-17A, IL-10, and TGF-β1. Both agents inhibit osteoclast differentiation and bone resorption; however, MTX is more effective at inhibiting RANK expression than YSJB alone. The combination aims to enhance efficacy for patients who do not respond adequately to monotherapy[2][3][5].
