JUUL is an electronic nicotine delivery system (ENDS) that utilizes a proprietary nicotine salt formulation in pre-filled pods to deliver nicotine via inhalation. In a clinical trial led by Butler Hospital, JUUL (specifically 5% nicotine pods) is being evaluated as a smoking cessation or harm-reduction tool for methadone-maintained smokers, a population with high smoking prevalence and significant barriers to quitting. The device operates by heating a liquid containing nicotine, benzoic acid, propylene glycol, and glycerol to create an aerosol, providing a rapid pharmacokinetic profile of nicotine that mimics traditional combustible cigarettes more closely than standard nicotine replacement therapies (NRT). The primary mechanism of action is the agonism of nicotinic acetylcholine receptors (nAChRs) in the central nervous system, which alleviates withdrawal symptoms and reduces the urge to smoke.
