JZQ-B4 nanobody-radionuclide conjugate is an engineered radiopharmaceutical platform designed to target mesothelin (MSLN) for cancer imaging and therapy. The construct utilizes a mesothelin-specific nanobody (JZQ-B4) that has been optimized to overcome the typical limitations of nanobody-based delivery, such as rapid renal clearance and kidney toxicity. To enhance its therapeutic window, the nanobody is engineered as a fusion protein with an albumin-binding nanobody to extend its circulation half-life from less than one hour to over 24 hours. Additionally, it incorporates specialized amino acid linkers between the chelator (e.g., DOTA or DFO) and the nanobody to facilitate renal clearance of the radiolabeled constructs, thereby reducing radiation nephropathy. The platform is versatile, supporting various radionuclides including Zirconium-89 for PET imaging, and Lutetium-177 or Actinium-225 for beta and alpha targeted radiotherapy, respectively.
