K-828 is an investigational oral small molecule developed by Kowa Co., Ltd. for the treatment of behavioral and psychological symptoms of dementia (BPSD) in patients with Alzheimer's disease. BPSD encompasses a range of non-cognitive symptoms, including agitation, aggression, anxiety, and psychosis, which significantly impact the quality of life for patients and caregivers. K-828 functions as a dual serotonergic modulator, acting as a partial agonist at the 5-HT1A receptor and an antagonist at the 5-HT2A receptor. This mechanism is hypothesized to stabilize serotonergic neurotransmission, providing anxiolytic and antipsychotic effects while minimizing the side effects associated with non-selective dopamine antagonists. The drug is currently in Phase 2 clinical development in Japan.
