K-877-ER is an extended-release formulation of pemafibrate, a highly potent and selective peroxisome proliferator-activated receptor-alpha modulator (SPPARMα). Developed by Kowa Research Institute, it is a small molecule drug designed to improve lipid metabolism and reduce hepatic inflammation and fibrosis. Unlike traditional PPARα agonists, pemafibrate is engineered for high selectivity to minimize off-target effects while maximizing therapeutic efficacy in metabolic and liver diseases. It is currently being investigated primarily for the treatment of nonalcoholic steatohepatitis (NASH) and liver fibrosis, often evaluated in combination with other agents such as the SGLT2 inhibitor tofogliflozin.
