K16ApoE is a synthetic cationic peptide carrier designed to facilitate the delivery of therapeutic agents across the blood-brain barrier (BBB). It consists of 16 lysine (K) residues linked to a segment of apolipoprotein E (ApoE) that binds to the low-density lipoprotein receptor-related protein (LRP). The peptide functions by inducing transient BBB permeabilization, primarily through the stimulation of endocytosis, allowing for the co-administration of small molecules (e.g., cisplatin, methotrexate, dabrafenib) and large proteins (e.g., tripeptidyl peptidase 1). While effective in preclinical models for conditions such as Alzheimer's disease, brain metastases, and late-infantile neuronal ceroid lipofuscinosis (LINCL), its clinical translation is currently limited by a narrow therapeutic index and potential acute toxicity, possibly involving acetylcholinesterase inhibition.
