Overview

KAD-1229 + insulin is a combination therapy consisting of **mitiglinide** (KAD-1229), a short-acting oral antidiabetic agent classified as a *meglitinide analog*, and exogenous **insulin**. Mitiglinide stimulates insulin secretion by selectively and potently inhibiting ATP-sensitive potassium channels (KATP channels; Kir6.2/SUR1) on pancreatic beta cells, leading to membrane depolarization, calcium influx, and subsequent insulin release in a glucose-dependent manner. When combined with insulin, this regimen targets both impaired insulin secretion and insulin resistance in patients with type 2 diabetes mellitus, potentially providing robust glycemic control through synergistic mechanisms. The combination is particularly studied for patients not adequately controlled on monotherapy, aiming to enhance early-phase insulin response and manage postprandial hyperglycemia[3][5][1].
