Kaihoujian is a traditional Chinese medicine (TCM) formulation originating from Miao medicine and is primarily used as a throat spray for the treatment of acute and chronic pharyngitis, tonsillitis, laryngitis, and related respiratory or oral diseases. The formula consists of several herbal ingredients including Sophorae Tonkinensis Radix et Rhizoma (Sophora tonkinensis), Ardisia crispa root (Ba Zhao Jin Long), Cicadae Periostracum (Chan Tui), and menthol. Its main active constituents include sophocarpine, matrine, bergenin, norbergenin, 11-O-galloylbergenin, anagyrine, calycosin and trifolirhizin. Pharmacological studies suggest that its anti-inflammatory effects are mediated through inhibition of inflammatory mediators such as NO production in macrophages and reduction of white blood cell count as well as inflammatory cytokines like IL-1β and MCP-1. Network pharmacology analyses indicate involvement with targets such as EGFR (epidermal growth factor receptor), MMP9/3/1 (matrix metalloproteinases), PTGS2 (COX-2) via PI3K-AKT/NF-κB/Toll-like receptor signaling pathways[4][7]. Clinical studies have shown efficacy comparable to budesonide for acute laryngitis with particular benefit in sore throat relief[3]. It is widely used in China for both adults and children; pediatric formulations exist under the name "Kaihoujian Throat Spray children's type" or KHJSC[2][6].
