Kamlanoflast (also known as K9 or Kamuvudine-9) is an orally bioavailable small molecule multi-inflammasome inhibitor developed by Inflammasome Therapeutics. It is part of the Kamuvudine class, which consists of modified nucleoside analogs designed to inhibit inflammasome activation without the mitochondrial toxicity or reverse transcriptase inhibition characteristic of traditional nucleotide reverse transcriptase inhibitors (NRTIs). Kamlanoflast specifically targets the NLRP3 and NLRC4 inflammasome complexes, effectively blocking the processing and secretion of pro-inflammatory cytokines IL-1β and IL-18. In the context of amyotrophic lateral sclerosis (ALS), the drug is intended to penetrate the blood-brain barrier and mitigate neuroinflammation, a primary driver of motor neuron degeneration. It has demonstrated the ability to reduce levels of neurofilament light chain (NfL), a key biomarker of axonal injury, in both preclinical models and human motor neurons. Kamlanoflast is currently being evaluated in Phase 1/2 clinical trials for ALS, including its inclusion in the biomarker-driven Healey ALS MyMatch trial, as well as for other neuroinflammatory and ophthalmic conditions.
