Kanglaite is an injectable lipid emulsion derived from the seed of Coix lacryma-jobi (Job's tears), a plant used in traditional Chinese medicine. [1, 5] It is approved and widely used in China for the treatment of various cancers, including non-small cell lung cancer (NSCLC), liver cancer, and pancreatic cancer, often in combination with chemotherapy or radiotherapy. [2, 9, 11, 12] It is also being investigated in the U.S. as a potential treatment for solid tumors and cancer-associated cachexia. [1, 2, 9] Kanglaite exhibits a multi-faceted mechanism of action. It induces cancer cell apoptosis (programmed cell death) by modulating proteins like Bax and Bcl-2, and halts the cell cycle at the G2/M phase. [1, 10] It also interferes with tumor angiogenesis, reverses multi-drug resistance by blocking P-glycoprotein pumps, and enhances the effects of chemotherapy. [1, 11] Furthermore, Kanglaite has immunomodulatory properties, stimulating the activity of the patient's immune system against the tumor. [1, 14] Studies have shown it can regulate the PI3K/Akt/mTOR signaling pathway and other cancer-related pathways.
