KB39 is an investigational **synthetic glycan** designed to modulate the **gut microbiome** and increase production of **propionate**, a short-chain fatty acid associated with favorable metabolic effects. The compound was identified through ex vivo screening of a large synthetic glycan library for high propionate-producing capacity. In preclinical studies, KB39 enriched propionate-producing gut bacteria and increased propionate biosynthetic genes, leading to reductions in serum total cholesterol, LDL-cholesterol, insulin, hepatic triglycerides, hepatic cholesterol, atherosclerosis, liver steatosis, and inflammatory measures in western diet-fed LDL receptor knockout mice. It is being explored as a microbiome-directed therapeutic for cardiometabolic disease states such as metabolic syndrome, insulin resistance, dyslipidemia, obesity, and related atherosclerotic and fatty liver phenotypes.
