Overview

KBL409 is a specific strain of *Lactobacillus acidophilus* developed as a live biotherapeutic product (LBP) for the treatment of chronic kidney disease (CKD). It functions by restoring mitochondrial energy metabolism and providing immunomodulatory effects. Preclinical studies indicate that KBL409 upregulates key mitochondrial regulators such as PGC-1α and AMPK, restores fatty acid oxidation and the TCA cycle, and improves mitochondrial respiration and ATP production. Furthermore, it attenuates the NLRP3 inflammasome pathway, reduces macrophage infiltration, and promotes M2 macrophage polarization, ultimately leading to decreased kidney fibrosis and improved renal function markers like serum creatinine and albuminuria. The drug also appears to strengthen the intestinal barrier and reduce systemic inflammation associated with intestinal dysbiosis.
