KBP-042 is a synthetic peptide drug classified as a dual amylin and calcitonin receptor agonist (DACRA). It was developed to address both type 2 diabetes mellitus and obesity by improving insulin sensitivity, reducing body weight, and controlling glucose levels. Mechanistically, it acts as an agonist at both the amylin receptor and the calcitonin receptor. Preclinical studies demonstrated that KBP-042 reduces food intake, induces sustained weight loss, improves glucose homeostasis, enhances insulin sensitivity, lowers hyperinsulinemia and hyperleptinemia, suppresses inappropriate glucagon response, reduces incretins GLP-1 and GIP levels, improves gastric emptying rates, alleviates leptin resistance and insulin resistance. In clinical trials in humans with type 2 diabetes or obesity (including those on metformin), it showed evidence of target engagement but was associated with dose-dependent nausea/vomiting at higher doses[1][3][5][6][7].
