Keishikajutsubuto is a standardized Japanese Kampo medicine (TJ-18) composed of seven herbal extracts: Cinnamon Bark, Peony Root, Jujube, Ginger, Glycyrrhiza, Atractylodes Rhizome, and Aconite Root. Traditionally used in Japan for the treatment of neuralgia, rheumatoid arthritis, and joint pain, it has gained clinical interest as a supportive therapy in oncology. Specifically, it is being evaluated for the prevention and management of chemotherapy-induced peripheral neuropathy (CIPN), particularly the acute and chronic neurotoxicity associated with oxaliplatin treatment in colorectal cancer. The formulation's efficacy is largely attributed to its ability to improve peripheral blood flow and modulate pain signaling pathways, including the inhibition of transient receptor potential (TRP) channels and the modulation of voltage-gated sodium channels by aconitine alkaloids. In clinical trials, it is often administered alongside additional processed Aconite root (Bushimatsu) to enhance its analgesic properties against cold-sensitive dysesthesia.
