Keto-analogs of essential amino acids (often referred to as keto acids) are a group of nitrogen-free precursors used primarily in the nutritional management of chronic kidney disease (CKD). In a randomized controlled trial conducted by Peking University First Hospital, these supplements were evaluated for their impact on insulin resistance and metabolic markers in patients undergoing peritoneal dialysis. The mechanism of action involves the transamination of the keto-analogs into their corresponding essential amino acids by utilizing excess nitrogen from urea, thereby reducing nitrogenous waste and uremic toxin production while providing necessary nutrients. Although the primary endpoint of improving insulin resistance (measured by HOMA-IR) was not significantly different between groups in the Peking University study, the intervention demonstrated secondary benefits in reducing systemic inflammation (hs-CRP) and improving the leptin/adiponectin ratio (LAR), suggesting potential metabolic and cardiovascular benefits in dialysis patients.
