Klotho-encoding messenger RNA is an investigational mRNA-based therapeutic designed to induce the endogenous production of the Klotho protein, a single-pass transmembrane protein primarily expressed in the kidney that functions as a circulating hormone with potent anti-aging, anti-fibrotic, and renoprotective properties. By delivering the mRNA sequence via lipid nanoparticles (LNPs) or other delivery systems, the therapy aims to restore Klotho levels which typically decline in chronic kidney disease (CKD), acute kidney injury (AKI), and with aging. The mechanism involves the translation of the mRNA by host cells into functional Klotho protein, which then enters the circulation to inhibit TGF-beta signaling, suppress oxidative stress, and modulate phosphate metabolism through its role as a co-receptor for fibroblast growth factor 23 (FGF23).
