Koeleria macrantha pollen is an allergenic extract derived from the pollen of the prairie Junegrass (*Koeleria macrantha*), a species native to Eurasia and North America. It is classified as a biologic product and is primarily used for the diagnosis and treatment of IgE-mediated allergic conditions such as allergic rhinitis (hay fever) and allergic asthma. For diagnostic purposes, it is used in skin prick or intradermal testing to identify sensitivity to grass pollens. For therapeutic purposes, it is used in allergen immunotherapy (AIT), where patients are administered gradually increasing doses of the extract to induce clinical and immunological tolerance. This process involves shifting the immune response from a Th2-mediated allergic profile to a Th1 and regulatory T-cell (Treg) response, reducing the production of allergen-specific IgE and increasing protective IgG4 antibodies. The product has been licensed in the United States since the late 1960s.
