KW-2450 + lapatinib + letrozole is an investigational oral combination therapy studied for advanced or metastatic hormone receptor-positive, HER2-positive breast cancer. KW-2450 is a small molecule dual inhibitor of insulin-like growth factor 1 receptor (IGF-1R) and insulin receptor (IR) tyrosine kinases, which are implicated in tumor cell proliferation and survival. Lapatinib is a small molecule reversible inhibitor of the intracellular tyrosine kinase domains of epidermal growth factor receptor (EGFR/HER1) and human epidermal growth factor receptor 2 (HER2), blocking downstream signaling pathways involved in cell proliferation. Letrozole is an aromatase inhibitor that suppresses estrogen synthesis, reducing hormone-driven tumor growth. Preclinical studies demonstrated synergistic antitumor effects when these agents were combined, leading to enhanced apoptosis and inhibition of cancer cell proliferation in HER2-positive and ER-positive breast cancer models. A phase I clinical trial established the recommended phase II dose for this triple-drug regimen in patients with advanced/metastatic hormone receptor-positive, HER2-positive breast cancer[3][4].
