Overview

L-000830982 is a small molecule GABA-A receptor modulator that selectively targets the alpha-1 subunit. Developed by Merck (MSD) and evaluated in collaboration with the Centre for Human Drug Research (CHDR), the compound was investigated as a potential symptomatic treatment for essential tremor. The development rationale was based on the observation that ethanol, which acts non-selectively on GABA-A receptors, effectively reduces tremors in patients. By specifically targeting the alpha-1 subunit, L-000830982 was designed to mimic the tremor-lytic effects of ethanol while potentially avoiding the broader sedative, cognitive, and addictive side effects associated with non-selective GABAergic agents. Clinical evaluation included a randomized, double-blind, placebo-controlled crossover trial comparing the effects of oral L-000830982 and intravenous ethanol on tremor severity and central nervous system function in outpatients with essential tremor.
