L-DOS47 is a clinical-stage antibody-enzyme conjugate (immunoconjugate) developed for the treatment of solid tumors, particularly those expressing Carcinoembryonic Antigen-related Cell Adhesion Molecule 6 (CEACAM6). It consists of a camelid-derived single-domain antibody that specifically targets CEACAM6, conjugated to urease derived from the jack bean plant. The mechanism of action involves targeted delivery of urease to tumor sites, where it enzymatically converts urea into ammonia and hydroxyl ions. This reaction increases the local pH in the acidic tumor microenvironment and generates cytotoxic levels of ammonia, leading to cancer cell death through both direct toxicity and reversal of immune-suppressive acidity. L-DOS47 is being investigated as both monotherapy and in combination with chemotherapies such as pemetrexed/carboplatin or doxorubicin for indications including non-small cell lung cancer (NSCLC) and pancreatic cancer[2][3][4][5][7][8][9].
