L-hydroxyproline (trans-4-hydroxy-L-proline) is a non-essential amino acid and a primary constituent of collagen, where it plays a vital role in stabilizing the collagen triple helix through hydrogen bonding. It is produced biologically via the post-translational hydroxylation of proline residues by the enzyme prolyl hydroxylase. While it is extensively used as a pharmaceutical intermediate in the synthesis of various drugs, most notably the HIV protease inhibitor atazanavir, it also has direct clinical applications. It is utilized as a diagnostic biomarker for conditions involving high collagen turnover, such as bone resorption and liver fibrosis. Therapeutically, l-hydroxyproline is incorporated into topical formulations to promote the repair of superficial wounds by supporting extracellular matrix synthesis. It is commercially produced through fermentation processes, with major suppliers including Evonik.
