L. casei SB27 polysaccharide is an exopolysaccharide (EPS) derived from the *Lactobacillus casei* SB27 strain, which was originally isolated from yak milk in the Gansu Tibetan area of China. This biologic compound is primarily composed of galactose and glucose and exists in two molecular weight fractions, LW1 (25.10 kDa) and LW2 (12.34 kDa). In preclinical models of colorectal cancer, the polysaccharide has demonstrated significant antitumor activity by inducing mitochondrial damage and apoptosis. Its mechanism of action involves the upregulation of histidine nucleotide binding protein 2 (HINT2) and the activation of the HINT2/mitochondrial calcium uniporter (MCU) signaling pathway, leading to increased calcium ion entry into mitochondria. This process triggers the expression of pro-apoptotic genes such as Bax, Bad, Caspase-3, and Caspase-8. Research is currently led by academic institutions like the Harbin Institute of Technology and Ocean University of China, with no current evidence of commercial development or human clinical trials.
